Steps in Internship Application
- For students who have not taken "Occupational Health and Safety" course from our department, they receive a "Occupational Health and Safety" certificate from an authorized institution, which is a minimum of 16 hours and includes the topics of the training given.
- If the workplace requires a compulsory internship form, the “Compulsory Internship Form” is filled. After it is signed by the internship commission, this form is delivered to the requesting company. "Compulsory Internship Form" is available at the end of the page.
- "Workplace Approval Form" is filled. The part that says “This part will be filled by the STUDENT” is filled by the student; the part that says "This part will be filled by the COMPANY/INSTITUTION" is filled by the workplace where the internship will be done, and the parts filled by the workplace are signed and stamped. "Workplace Approval Form" is available at the end of the page.
- "Workplace Information Form" is filled. This form is filled in completely by the workplace where the internship will be done, and the parts filled by the workplace are signed and stamped. "Workplace Information Form" is available at the end of the page.
- Students make internship applications from the internship application tab on OBISIS at least 10 days before the start of the internship. When he/she completes his/her application, he/she prints out and signs the document that OBISIS produces as a PDF.
- The student who completes these procedures must personally submit the following documents to the relevant commission member: Internship Application Document received and signed as a result of the online application via OBİSİS, Workplace Approval Form, Workplace Information Form and Occupational Health and Safety Certificate for students who have not taken the Occupational Health and Safety course from our department. Submission of documents and internship applications via e-mail or post will not be accepted and will not be taken into consideration. If the applicant student is out of town, they must submit it directly to the internship committee members through a friend.
- The application, which is approved as "suitable for internship", is also approved online by the relevant commission member. If the student is late in submitting their documents (the documents must be submitted ten days before the start of the internship), the system will not allow the commission member to approve the application, since deadline for application approval will pass. In this case, the student will have to contact the workplace and make a new application.
- The student whose internship application is approved by the commission member prints out "Internship Registration Form" and "Insurance Initial Document" to be given to the workplace via OBISIS. "Insurance Initial Document" link is active at the start date of the internship at OBİSİS.
- Students will only be able to consult with the relevant internship commission members, submit documents and have their compulsory internship form signed during working hours on the days listed below. However, if they cannot reach the relevant commission member, they can go to other commission members.
Monday: Res. See. Dr. Burak ULU, Member of Commission
